Apparently it’s from Belgium, and yes, there is an indication of a third child. My guess is that it’s a girl, topless, and so not so great for less liberal American tastes. Less liberal not being a political agenda, but against nudity or semi-nudity, even for children. I’m sure Mom instigated this. If I looked like this, I’d market in this outfit..

Actually, I think the leaves and coconut bra ARE photoshopped. Look at them closely. Possibly the Santa hats, too. I think the original photo was of a run-of-the-mill nude family and the, uh, trimmings were added.
